Emergency Health Care Form (Spanish)
Immunizations
Immunizations are required in keeping with the regulations of the Commonwealth of Massachusetts.
Immunization clinics are set up in the school to meet these requirements. [KB Question: What kind of authorization is required from the parents? Is there a permission form available for them to fill out? What is the procedure for taking advantage of the clinic? When are the clinics held?]
Normal Procedure for Going to the Nurse
Obtain a signed, dated and timed pass from the instructor to whom you have been assigned. Proceed directly to the nurse with all your belongings. If the Health Office is closed, report directly to the High School office.
Medications Policy
When the need arises for dispensing medications during the school day, the following procedure will be in effect:
A. Over-the-counter Medications
A written, signed, and dated request should be made by the parent or guardian requesting the administration of medication to his/her child during sdlool hours. Tylenol may be dispensed by the nurse for headache, cramps, etc. only with the written permission of parent or guardian.
B. Prescription Medications
State regulations govern the administration of prescription medications during school hours. These regulations also govern the use of inhalers carried by many students. These regulations require signed medication orders by health care providers and signed consent forms by parent or guardian.
Please contact the school nurse if your student will require any prescription medication during the school day.
All prescription medication must be delivered to the school in a container labeled by your pharmacist or health care provider.
Emergencies
In the event of sudden, serious illness or accident requiring medical attention, and the parents cannot be reached, the student will be taken to the nearest emergency facility (by ambulance, if necessary) and the parent will be responsible for all medical costs incurred. For this reason it is important to have emergency numbers where the school can reach the parent or their designate during school hours.
